INSTRUCTIONS FOR PAYMENT REQUEST

(See section 5.1 of the Procedure Manual)

  1. Model 17 of payment request
  2. Explanatory account containing:
    • An explanatory activity report.
    • A financial report containing:
      1. An itemized account of expenditures and investment activity, identifying the creditor of the document, the amount, date of issue and date of payment.
      2. Invoices or documents of probative value and payment documentation.
      3. Certificate of independent appraiser who is duly accredited and registered with the pertinent official registry, in the case of property acquisition.
      4. A detailed account of other sources of income or grants that have financed or subsidized investment activity indicating the amount and origin
      5. The three budgets stipulated under Article 31.3 of the General Subsidies Act should have been requested the individual recipient or entity.
  3. Updated supervisory statements . Model 2 , Model 3 , Model 4 and Model 5
  4. Certificates verifying an up-to-date status on Social Security payments as well as regional and state treasury payments.
  5. To justify the creation and / or maintenance of employment, a copy of the employment contract must be provided along witha copy of the TC1 and TC2 . In the case of self-employed workers, proof of registration as self-employed or employment history report.
  6. Verifying documentation of the property or possession with an official title of the place where the investment is set to be made, if deemed necessary and the documentation was not submitted during the application stage.
  7. Construction permit and operating license if applicable.
  8. Any other permit or necessary for conducting activity.
